Musqueam band members must submit authorization forms by Friday, April 4 at 4:30pm to receive distribution on Tuesday, April 15.

All members over 19 years old must complete only ONE authorization form.
FORM A: Individual Authorization
This form is for Musqueam members (aged 19+) to authorize their personal distribution payment by direct deposit or cheque.
Parents who are also authorizing their Musqueam children’s payments DO NOT complete this form. Instead, Form B1 replaces Form A.
FORM B1 & B2: Family Authorization
This two-part form is for Musqueam members (aged 19+) to authorize their personal distribution payment AND their Musqueam children’s distribution payment.
This form is also for non-Musqueam parents or guardians of Musqueam minors to authorize their children’s distribution.
There are two parts to this form: B1 and B2.
Form B1: Individual Authorization
- Completed by ALL parents or guardians of Musqueam children.
- If you are a Musqueam member, Form B1 replaces Form A.
Form B2: Guardian’s Acknowledgement – Children’s Property
- List of children and payment authorization on behalf of children.

- Drop off to the band office during operating hours (Monday to Friday, 8:30am to 4:30pm, closed 12pm-1pm).
- Do not leave it in the social assistance mailbox!
Deadline for newborn registration with the membership registrar for eligibility to receive distribution is April 4, 2025.
Forms submitted after April 4 cannot guarantee a payment on April 15.
Late forms are processed after initial payments are processed, and payments are issued after April 15.
